A CASE REPORT OF MULTIPLE BLEEDING PSEUDOANEURYSMS COMPLICATING ACUTE PANCREATITIS WITH RENAL CELL CARCINOMA AS AN INCIDENTAL FINDING

The development of pseudoaneurysms is a rare but high-mortality complication of acute pancreatitis, making timely diagnosis and management crucial. Imaging modalities such as ultrasonography and computed tomography (CT) scans, used to diagnose and assess disease severity, can also detect asymptomatic incidentalomas. This case report describes a rare occurrence of multiple bleeding pseudoaneurysms complicating acute pancreatitis, with renal cell carcinoma as an incidental finding. A 50-year-old man with a history of stroke presented with epigastric pain. On diagnostic workup, he was found to have acute necrotizing pancreatitis, gallstones, and a suspicious right renal mass, which was shown on biopsy to be a clear cell carcinoma. After initial improvement, he suddenly developed hematemesis secondary to bleeding pseudoaneurysms of the gastroduodenal, pancreaticoduodenal, and splenic arteries. Management used a combined approach with an emergency exploratory laparotomy and endovascular coiling. This case report highlights the importance of early pseudoaneurysm detection and how a timely, well-coordinated multidisciplinary approach can help manage complicated cases with multiple simultaneous pathologies.
