Abstract

Objective: To find the association of hormonal and clinical abnormalities among obese and non-obese women and to compare hormonal and clinical abnormalities between obese and non-obese women attending a tertiary care hospital.

Materials and Methods: After ethical approval, we conducted a descriptive cross-sectional study at the Tertiary Care Hospital in District Peshawar, Pakistan, from April 2024 to March 2025. We selected 200 samples using convenience non-probability sampling. We collected data using a validated structured questionnaire and analyzed them using SPSS version 24. We used the chi-square test to assess statistical association.

Results: In our study, 58.50% were under 30 years old; 38.50% were obese; 23.50% had high socioeconomic status; 39.50% were illiterate; 68% were urban residents; and 37.5% were physically inactive. Among obese PCOS women, 41.56% had prolonged periods (> 7 days); 59.74% had oligomenorrhea (< 3 days); and 77.92% had amenorrhea. Furthermore, among obese PCOS women, 71.43% had acne; 84.42% had hair loss; 50.65% had diabetes mellitus; 72.73% had infertility; 71.43% had disturbed sleep; and 58.44% had anxiety. Among non-obese women, 32.52%, 44.72%, 56.91%, 47.97%, and 38.21% had diabetes mellitus, infertility, disturbed sleep, fatigue, and anxiety, respectively (P value < 0.05).

Conclusion: Menstrual irregularities, including prolonged and partial absence of periods, were significantly associated. Moreover, acne, hair loss, diabetes mellitus, fertility disorders, disturbed sleep, and mood disorders/anxiety also showed significant associations among obese and non-obese PCOS women; thus, early detection and awareness of PCOS were needed.
